Please print and return this sheet to us with your $100 deposit if you have not already paid it.  All information will remain strictly confidential.  Hunting dates are confirmed upon receipt of deposit. 

Please note that every hunter (including out-of-state hunters) born on or after September 2, 1971, must successfully complete a Hunter Education Training Course.  A Special Resident Hunting License must be obtained for residents and non-residents that fall under this age requirement.
